Preliminary clinical application of domestic endoscopic surgical robotic system in partial nephrectomy
Objective To investigate the safety and efficacy of the Condor endoscopic surgical robotic system in partial nephrectomy. Methods This study included patients with T1 renal tumors who were eligible for enrollment in Peking University First Hospital from December 2020 to February 2021. Inclusion criteria: ①Patients requiring partial nephrectomy; ②The tumor TNM stage was T1; ③R.E.N.A.L. score≤9 points; ④Age 18-75 years old. Exclusion criteria: ① patients with uncontrolled serious diseases or acute infection; ② patients with cardiovascular and cerebrovascular diseases, blood system diseases or diabetes that cannot be controlled and cannot meet the surgical standards; ③ patients with immune system diseases that cannot be controlled and cannot meet the surgical standards ④ Pregnant or lactating women. All patients underwent partial nephrectomy using the Condor endoscopic surgical robotic system. The preoperative, perioperative and follow-up data of the patients were collected. Results A total of 26 patients were included in this study, including 16 males and 10 females; the median age was 53 (33-74) years old; the median body mass index was 25.99 (20.90-32.91) kg/m2; 12 had left kidney tumors, and 12 had right kidney tumors. There were 14 tumors; the median tumor diameter was 2.2 (1.0-3.5) cm. All operations were performed according to the established surgical plan to remove the diseased tissue. The median warm ischemia time was 17.7 (7.1-29.2) min, the median device docking time was 4.7 (2.3-9.9) min, and the median operating time of the robotic arm was 65 (37-29.9) min. 155) min, the median NASA-TLX score was 5.3 (2.0-28.0) points, and no device-related adverse events occurred during the operation. The median postoperative hospital stay was 4 (4-5) days. Postoperative pathological examination showed that all tumor margins were negative. There was no Clavien ≥ grade II surgical complication in all patients during the perioperative period and 1 month follow-up. Conclusion Partial nephrectomy using the Condor endoscopic surgical robotic system has no adverse events related to equipment and surgical complications. The system is safe and effective for partial nephrectomy.
