12-11-2022

The clinical data and endoscopic mucosal injury of 71 children with abdominal HSP (42 males and 29 females, aged 3-15 years) who were hospitalized in Anhui Children's Hospital from November 2016 to December 2019 were analyzed. and pathological features. Results 45% of the children had gastrointestinal symptoms before the purpuric rash. Under gastroscopy, the mucosa of esophagus, stomach, and duodenum have different degrees of damage, including mucosal erythema, congestion, edema, punctate or strip-like erosions, submucosal or mucosal bleeding, and in severe cases, shallow concave ulcers. The descending duodenum was most involved and the severity was the most serious, followed by bulb, gastric antrum, gastric fundus and gastric body. The esophagus and gastric angle are rarely involved, and the degree is mild. Histopathological features were non-specific inflammation, showing erosions, lymphoid tissue hyperplasia, and inflammatory cell infiltration. Conclusion The gastrointestinal symptoms of HSP in children are diverse, and the characteristic changes of mucosa under gastroscope can assist in the diagnosis, but the endoscopic indications should be strictly controlled to avoid increasing the pain of children and the burden on the family.
