Clinical analysis of 62 cases of pituitary adenoma resection by transnasal sphenoidal approach under neuroendoscopy

Objective To summarize and analyze the efficacy and application value of transnasal sphenoidal neuroendoscopic resection of pituitary adenoma. Methods 62 patients with pituitary adenoma admitted to our hospital from January 2017 to September 2019 were retrospectively analyzed, including 30 males and 32 females. The patients were aged from 27 to 81 years, with an average age of 52.6 years. Pituitary adenomas included microadenomas in 10 cases, large adenomas in 38 cases and giant adenomas in 14 cases. The patient underwent neuroendoscopic transnasal sphenoidal tumor resection, followed up by neuroimaging, endocrine, and visual examination. Results The follow-up lasted from 1 to 34 months, with a median of 17 months. Among the 62 patients, total resection was performed in 50 cases, subtotal resection in 9 cases and major resection in 3 cases. Postoperative endocrine cure rate 34.6%, visual acuity improvement rate 92.3%, headache improvement rate 83.3%. There were 12 cases of transient diabetes insipidus, 5 cases of transient cerebrospinal fluid rhinorrhea, and no permanent diabetes insipidus or cerebrospinal fluid rhinorrhea. No intracranial infection, coma, plant survival or death and other serious complications. Conclusion Transnasal sphenoidal neuroendoscopic resection of pituitary adenoma is a safe and effective treatment.
