Analysis of the curative effect of high frequency pulse irrigation on gout arthritis of knee joint under arthroscope

Objective To investigate the effect of high frequency pulse irrigation on gouty arthritis of knee joint under arthroscope. Methods A total of 79 patients with knee gouty arthritis were admitted to our department from March 2007 to January 2019, including 69 males and 10 females. The age was (40.6±10.1) years (21-52 years). The patients were divided into pulse group and control group according to surgical methods. In the pulse group, 44 patients were treated with high frequency pulse to clean gout crystals on the basis of traditional arthroscopy. 35 patients in the control group were treated with arthroscopic planing knife and curette to remove gout crystals. The general operation conditions of the two groups were recorded. The cleaning success rate of pulse group was calculated. Postoperative blood uric acid and C-reactive protein levels, number of acute systemic gout episodes, visual analogue Scale (VAS) score, the Western Ontario and McMaster Universities were recorded Universities Arthritis Index,WOMAC), Kellgren-Lawrence scale. Results The total operation time, articular cartilage and meniscus cleaning time, VAS score and WOMAC index of 1 week and 2 years after operation in pulse group were significantly lower than those in control group, and the differences were statistically significant (P<0.05). The acute attack rate of systemic gout in pulse group (4.5%) was significantly lower than that in control group (22.9%), and kellgren-Lawrence grading was significantly better than that in control group, with statistical significance (P<0.05). In the pulse group, the success rate of articular cartilage and meniscus was 97.7%(43/44), but the success rate of cruciate ligament and synovial membrane was only 6.8%(3/44) and 15.9%(7/44). Conclusion High frequency pulse irrigation under arthroscopy has the advantages of simple operation, high efficiency and little damage in the treatment of gout arthritis, especially for the cleaning of articular cartilage and meniscus, which can be used as an auxiliary means of routine cleaning.
