Larynoscope surgery operation

The surgeon stands on the client's head and protects the upper incisor with gauze. Hold the direct laryngoscope in the left hand, insert the tube into the larynx and pharynx along the back of the tongue, stir up the epiglottis, expose the glottis, examine the bilateral arytenoid area, ventricular band, laryngeal room, vocal cord, glottic and subglottic area successively, and ask the patient to make sound, observe the movement of bilateral vocal cords. When laryngeal lesions are seen, different surgical methods should be used according to the situation:

1. Vocal cord polyp

(1)There are pedicled polyps at the edge of vocal cord, and a throat forceps can be selected to remove the polyp, and pay attention to not damage the vocal cord during the operation;

(2) Wide base polyp of vocal cord edge, with triangular warped forceps along the vocal cord edge to remove it, pay attention to bite less, not too deep, because vocal cord injury can cause permanent hosing;

(3) The polyps at the front of the vocal cords should not be damaged when removing the polyps with triangular forceps, otherwise it will cause adhesion at the front of the vocal cords and cause permanent hosing.

2. Cyst of vocal cord can use laryngeal knife to cut mucous membrane, separate slightly, with cup-shaped laryngeal forceps open forceps mouth, press vocal cord lightly, often can remove cyst completely.

3. Vocal cord nodule The smaller the lesion on the vocal cord, the more careful the operation. Hold the knot with a throat clamp, pull it towards the center, and then bite it off. Resection of vocal nodules is best performed under surgical microscope.

4. The tumor of laryngeal papilloma was removed by laryngeal forceps, and fluorouracil was injected into the base of the papilloma, which was effective in preventing recurrence. It is also possible to scrawl oil on the base of the tumor.

5. Biopsy should be performed if malignant tumor or lesion of unknown nature is suspected. A biopsy should be performed prior to a careful examination of the larynx to determine the extent of the lesion and movement of the vocal cords. The biopsy causes bleeding, which can prevent necessary tests. Biopsy site should be selected at the junction of pathological tissue and normal tissue to avoid biopsy at necrotic tissue.
